Informed consent and waiver of liability: 

I understand that I am enrolling in the CrossFit Franco's Gulf Coast Games fitness competition in Mandeville, LA. I fully acknowledge the activities here will include strenuous physical activity, including, but not limited to, muscle strength, and endurance activities, cardiovascular conditioning activities and other physical activities included as a part my participation at CrossFit Franco’s Gulf Coast Games. 

In order to be admitted to participate in physical activity at CrossFit Franco's, I have represented to, and do hereby confirm, that I am in good physical condition and that I do not suffer from any known condition that would otherwise limit, or put in risk my participation at CrossFit Franco's Gulf Coast Games, and/ or put myself to high risk for harm. I further acknowledge that the released Parties as hereafter identified rely upon my good health representation when allowing me to be admitted to this gym. 

 In consideration of my admission to CrossFit Franco's Gulf Coast Games and my participation, I do hereby release and forever discharge CrossFit Franco's, FitCorp, and all their affiliates, subsidiaries, employees, coaches, subcontractors, volunteers, and representatives from any and all claims, demands, and causes of actions which may arise from my participation in this event. 

I fully understand that I may injure myself as the result of my participation in CrossFit Franco's Gulf Coast Games, and which injury may include heart attacks, muscle strains, pulls, tears, broken bones, heart prostration, and miscellaneous injuries to the back, foot, and other areas of my body. I also confirm that my participation is fully voluntary on my part and I may, and have been instructed to, cease my physical activity upon the occasion that I experience any physical condition which may compromise health. 

I hereby affirm that I have read the entirety of the above and foregoing consent and waiver, understand all of the terms and conditions hereof and do hereby voluntarily sign this document. 

Refund policy: 

No refunds will be given once an event spot has been paid for. 

Image release: 

CrossFit Franco's Gulf Coast Games and it's parties have the right to use participants likeness, image, and appearance, to record or transfer to photographs for use in print media, or social media to be used to exhibit, and/ or market the event.
